What you share

The form asks for your name, email address, enquiry type and message. You may also include your clinic or organisation. When you enquire about a current role, the role reference and title are included with your message. It records your permission to use those details, along with submission details that help us manage enquiries.

How the information is used

Your details are used to review your enquiry or interest in an opportunity and discuss it with you. Enquiries are processed and stored through Netlify Forms for access by En Medical’s authorised team. They are not displayed publicly.

Please keep patient information out of this form

This form is for career and doctor opportunity enquiries, clinic succession, investment and other business conversations. Please do not include medical records or sensitive patient details. For appointments or care enquiries, contact Procare GP directly.

Questions about a submission

To ask about your enquiry or request a correction or deletion, use the business enquiry form and include the approximate date and subject of your earlier enquiry. Please use the same contact email so the request can be verified.
